Hospital Admission Forms

My hospital mailed me the admission forms for my hospital stay next week.

The first sheet of paper in the packet explained the purpose of the packet and asked me to return the packet a week before my operation.

The admission form was one double sided paper that, for the most part, wanted to know how I was going to pay for my stay. To that end, it wanted to know my employer, wife's employer, and all of my insurance information.

Another sheet in the packet asked for my ethnicity. I thought that was odd, but... OK.

Another sheet said I would be included in the hospital directory. If I didn't want to be in the directory? I needed to tell admissions. 

That was basically it. Packet is completed and in the mail!
